Arguably, the biggest distraction in development is the disproportionate importance given to the idea of designing programs or schemes to address specific problems as the primary route to development and economic growth.

I think your observation 1 & 2 - political ownership and bureaucratic ownership respectively, are pretty much the reasons for development getting reduced to program design and ideas. Both of them are very tough to improve/address/fix etc. Piecemeal is the adjusted equilibrium. I don't know how to solve for it. But the beast of programmatic approach is partly the creation of bureaucracy and politicians, in that order, in the first instance.
